Chronic Disease Manager is a software application for all
medical clinics, hospitals or other medical institutions that need to
track chronic disease patients. CDM monitors chronic diseases and
enhances the quality of outcomes.

reports showing the trends of chronic diseases among your patient
population or break it down by an individual patient. CDM can track

and diabetes.
